Exemestane is an irreversible, selective aromatase inhibitor which acts as a false substrate for the enzyme and forms an intermediate that binds to its active site. This leads to inactivation or suicide inhibition, thus preventing the conversion of androgens to oestrogens in peripheral tissues. It lowers circulating oestrogens in postmenopausal women  breast cancers where growth is oestrogen-dependent.

Indications and Dosage:

Adjuvant therapy for postmenopausal women with hormone receptor positive early breast cancer

Adult: Following 2-3 yr initial adjuvant tamoxifen treatment, 25 mg once daily for a total of 5 yr of combined sequential adjuvant hormonal therapy; or earlier if tumour relapse occurs.

Advanced breast cancer in postmenopausal women

Adult: 25 mg once daily continued until evident tumour progression.

Exemestane is indicated for the adjuvant treatment of postmenopausal women with oestrogen receptor positive invasive early breast cancer (EBC), following 2 – 3 years of initial adjuvant tamoxifen therapy.

Exemestane is indicated for the treatment of advanced breast cancer in women with natural or induced postmenopausal status whose disease has progressed following anti-oestrogen therapy. Efficacy has not been demonstrated in patients with oestrogen receptor negative status.

The recommended dose of Exemestane is one 25 mg tablet to be taken once daily, preferably after a meal.

In patients with early breast cancer, treatment with Exemestane should continue until completion of five years of combined sequential adjuvant hormonal therapy (tamoxifen followed by Exemestane), or earlier if tumour relapse occurs.
